RMA Form is a document used in order to return a product, received a refund, request for a replacement, or have the product repaired. This form can be used as long as the product is still in warranty or it depends on the terms and agreement with the customer.
Place product(s) to be exchanged in a padded envelope along with this RMA form and proof of purchase (Online order number or store Receipt). It is recommended that you return your package to the address indicated above via a traceable method. Please allow 1-2 weeks for processing.
